A Building/Facility Request Form is required for all events, table setup etc. on the Southern University and A&M College campus. A Building/Facility Request Form must be submitted 7 days prior to event or your event will be cancelled without further notice.

Student Organization Renewal Form
All student organizations on the Southern University and A&M College campus must submit an organization renewal form annually. Organizations are required to have and maintain a President, Vice-President, Treasurer, and Secretary; with each being required to have and maintain a 2.5 Cumulative Grade Point Average during their tenure.

Student Organization Event Evaluation Form
All student organizations are required to submit an event evaluation form to the Coordinator for Student Organizations and Campus Involvement within 7 business days of the date of the event/program. Only students may submit this form for review.
