Document Submission Process:

 

New First-Time Student Applicants:

Submitting Missing Documents:

  • Jaguar Student Portal: Submit your information through the Jaguar Student Portal at https://apply.subr.edu/apply/.
  • ACT/SAT Score Reports:
    If you did not select Southern University to receive your ACT or SAT scores during registration, you must request an official score report directly from the respective testing website. Use the following school codes:
  • ACT: 1610
  • SAT: 6663

Official test scores are required to create your course schedule.

Final High School Transcripts:
Final transcripts must be sent directly from your high school counselor. They can be submitted electronically via the Jaguar Portal at https://apply.subr.edu/apply/ or mailed in a sealed, unopened envelope to:

Transfer Student Applicants:

All documents should be submitted via the Jaguar Student Portal at https://apply.subr.edu/apply/.  Official transcripts must be received from every institution attended.  The institution must submit the transcript directly to the Office of Admissions and Recruitment via email, electronically via Parchment, or via regular mail at the address above.

 

**Transcripts forwarded directly to the transfer counselor will not be considered for processing. Transcripts sent via the Black Common App will not be deemed official-per the Louisiana Board of Regents. Please request an OFFICIAL transcript from the institution or through the Clearing House.**



Shot Records:

All accepted students must satisfy immunization verification requirements:
  1. Log in to Med+Proctor using your SUS student email address and password, select "Account" and create a profile by completing all required information.
  2. Download the immunization certificate from Med+Proctor for immunization verification requirements, and follow the steps to complete and submit.
  3. Connect with Med+Proctor to help you through the process via online chat, knowledgebase or by submitting questions.
  4. Please allow 2-3 business days for processing and verification of all immunization documents after submission.
  5. Be sure to check your SUS student email for all verification updates from Med+Proctor.
